Department of Vascular Surgery

Vascular surgery is the medicine of the future, it can save patients from the serious consequences of vascular disease: thrombosis, thrombophlebitis, veinous ulcers, amputations, acute renal failure, stroke, bursting, etc.

If there is a feeling of heaviness in the lower limbs, swelling, headaches, tinnitus, loss of consciousness, spider naevus on the legs or if the skin colour has changed markedly, you should visit a doctor specialising in vascular medicine.

The Department of Vascular Surgery at Multidisciplinary city hospital №1 in Astana performs the entire spectrum of operations and endovascular manipulation on the branches of the aortic arch, the abdominal aorta and its visceral branches, as well as the main arteries and veins of the extremities.

Surgery on the aorta and its branches and peripheral arteries is conducted both open pit and using minimally invasive procedures which permit medics not only to shorten and ease the recovery period of patients, but also to retain a good cosmetic effect, which is very important in operations on the great vessels of the lower limbs.

Along with the traditional methods of treatment of varicose disease, if necessary we use methods of sparing surgery which also have undoubted therapeutic and aesthetic value.

Our leading experts have been trained in clinics in Russia, Turkey, India and China.

The department provides services both within the scope of guaranteed free medical care, or for a fee. Highly specialised medical aid is free.

MAIN AREAS OF WORK

Diseases of the aorta and major arteries:

  • atherosclerosis with lesions of various segments of the aorta (the descending thoracic, supra - and intra infrarenal), brachiocephalic arteries, visceral and renal arteries, the arteries of the pelvis and lower extremities
  • non-specific aortoarteritis
  • thromboangiitis obliterans
  • fibre-muscular dysplasia
  • aneurysms of different localisation (excluding intracranial, the ascending portion of the thoracic aorta and arch)
  • arterioveinous fistulas
  • urgent conditions: vascular injury, thrombosis and embolism

Veinous diseases:

  • varicose veins, including complicated forms
  • post-thrombotic disease (post-trombophlebitic syndrome)
  • secondary varicocele associated with veinous hypertension of the left kidney

Emergency situations (for residents of Astana)

  • complicated abdominal aortic aneurysm and peripheral arterial vascular injury, thrombosis and embolism

The department is equipped with modern equipment which allows doctors to assess the blood flow in different anatomical areas, and to visualise the analysed blood vessel, its walls, the presence of atherosclerotic plaque, thrombotic masses. It is also possible to assess the condition of the valvular apparatus of superficial and deep veins.

Research:

  • Doppler ultrasound (triplex scanning)
  • multislice computed  tomoangiography (MSCTA)
  • Magnetic Resonance Angiography (MRA)
  • radiopaque coronary angiography, coronary angiography
  • veniography

Reconstructive and palliative surgery performed in the Department of Vascular Surgery:

  • carotid endarterectomy
  • reconstructive surgery on the aortic arch branches
  • resection of the coarctation of the aorta with plastic surgery
  • resection of abdominal aortic aneurysm with prosthetics
  • peripheral arterial aneurysm resection
  • aorto-femoral bypass : a one-sided (issued), bifurcation (ABBSH)
  • ilio-femoral bypass (SHPD)
  • femoropopliteal bypass (BPD)
  • femoral-tibial artery bypass grafting (HSP) and other intervention at the level of the arteries of the leg
  • endarterectomy of the femoral arteries  profundoplasty
  • operations on paired and unpaired visceral branches of the abdominal aorta (renal artery, celiac trunk, superior mesenteric artery and inferior mesenteric artery)
  • correction of vasculogenic erectile dysfunction
  • repeated reconstruction (thrombectomy with shunts, replacement grafts, resection of aneurysm, reconstruction anastomosis)
  • extra-anatomic bypass
  • scalenotomy, resection of additonal cervical extension or I-rib with the syndrome out of the chest
  • traditional upper-thoracic and lumbari
  • Endoscopic thoracic and lumbar sympathectomy in Raynaud's phenomenon, the distal arterial bed
  • amputation and disarticulation
  • formation of arteriovenous fistula, vascular graft implantation in patients with chronic renal failure (CRF)

Surgery for veinous diseases:

  • traditional phlebectomy
  • minimally invasive operations: miniflebektomiya, stem scleroobliteration, endovenous ablation laser diode
  • operation for Post-Thrombophlebitic Syndrome (PTS) extravascular correction valve of incompetent vein valves; valvuloplasty; uncoupling incompetent perforated veins on Kokketu, Linton; endoscopic subfascial dissection of incompetent perforator veins (SEPS)
  • imposition of draining auricular - jugular shunts for superior vena cava syndrome
  • secondary varicocele correction due to veinous hypertension of the left kidney (forming one - bidirectional testicle - iliacal anastomoses)

Combination of X-ray and endovascular  operations performed in the Department of Vascular Surgery:

  • angioplasty and stenting of the brachiocephalic arteries
  • angioplasty and stenting of peripheral arteries
  • angioplasty and stenting unpaired visceral branches of the abdominal aorta (celiac trunk, superior mesenteric artery) and renal arteries
  • implantation of the stent graft aneurysm of thoracic and abdominal aortic
  • cava filter implantation
  • Endovascular destruction of the adrenal hyperplasia
  • embolisation of arterioveinous fistulas, haemangiomas
  • varicocele embolisation
